What is a Personal Health Record (PHR)?

Personal health records (PHRs) are secure online services to help you take control of your own health.

You can use a PHR:

  • Like a daily organizer for health care:  E-mail doctors, make appointments, get test reminders and results, and refill prescriptions.
  • To keep track of all your heath information:  Keep everything, from important medical records to your over-the-counter medicine lists, in one place.
  • For help with personal goals:   Losing weight, controlling high blood pressure, or keeping track of fitness activities.
  • To manage health care for a family member:   Whether they live nearby or across the globe.
