HMO Guide for Seniors

The HMO Guide for Seniors focuses more on topics and concerns that are expressed by seniors to ensure that a high quality level of HMO health care service is experienced. It outlines ways to maximize effective communication with both the doctor and loved ones concerning the choice of treatment. It also covers the benefits most needed by seniors, and provides the tools to address any areas where care or customer service is lacking.

Health Care Quality Report Card (Summary)

The Health Care Report Card Summary is a 9” x 7” promotional piece for the Report Card that gives a brief description of the report card, displays the HMO star chart and the top 40 medical groups. The summary is used for those who want just the basic information and may not have access to the internet for the full Report Card on our site.

Health Care Quality Report Card (Booklet) (PDF)

The Health Care Quality Report Card Booklet is an 8½” x 11”, 42 page booklet with the HMO Ratings, several clinical measures such as Asthma Care, Diabetes Care, Mental Health Care, and more. This booklet also contains information about linguistic services and scoring and rating methods used to create the report card. The booklet is used for those who want a more in depth look at the different measures on the Report Card and my not have access to the full Report Card on our site.

HMO Health Center (HHC) Insert Card [English] [Chinese] [Korean] [Vietnamese] (PDF)

The HHC Insert Card is a two page card that fits in the back pocket of both the HMO Guide and the HMO Guide for Seniors. This educational piece provides an at-a-glance look at the services the HHC provides to Californians. It contains information on the Independent Medical Review Process (IMR), the most common problems patients have and also provides up-to-date contact information. The HMO Insert card is available in English, Spanish, Chinese, Vietnamese and Korean languages.

Much like the HHC Insert Card, the Mental Health Insert Card is designed to fit in the back pocket of OPA's Guides. In collaboration with the Mental Health Association in California and UC Berkeley, Health Rights in Action, this piece delivers helpful and current information about accessing mental health services in California with topics such as: What does your Health Plan Cover? Questions to ask, Choose a Mental Health Specialist, What Treatment is Right for You?, and more.

This is a tri-fold brochure that provides an overview of the Department of Managed Heath Care's HMO Help Center. This educational piece provides a glance at how the HHC can help patients by providing examples of formal complaints, the complaint process, and how to file a complaint. This brochure also contains up-to-date contact information so that California patients can get questions about their Health Plans answered by knowledgeable staff.

Independent Medical Review (IMR) Brochure [English] [Spanish] (PDF)

This is a tri-fold brochure that details the functions of the Department of Managed Health Care's HMO Help Center IMR services. This educational piece provides an overview of the IMR process. Topics include: examples of eligible appeals, who is and is not eligible, and an overview of the IMR process. Also included is up-to-date contact information so that California patients can get questions about their appeals answered by knowledgeable staff.
